How to get a CVOR abstract in Ontario

Before you start: what you need

The order form asks for a little more than a regular driver abstract does, so gather these first.

• Your full name exactly as it appears on your licence. A mismatch is the most common reason a request fails.
• Your Ontario driver’s licence number. Fifteen characters: one letter followed by fourteen digits, printed on the front of the card.
• An email address for delivery of the PDF.
• Your country, province or state, and postal or ZIP code.
• A payment card. Payment is handled by Stripe.

Make sure it is the driver abstract you need

Three different documents carry the CVOR name, and only one of them is about you as a driver. If a carrier or insurer asked you for a CVOR record, confirm which one before ordering.

• CVOR Driver Abstract. The five-year record of an individual driver’s commercial-driving events. This is the document covered by this guide.
• CVOR Level 1. A public summary of a commercial carrier’s safety record, not a driver record.
• CVOR Level 2. A more detailed carrier-only record with a different ordering process.

Ordering online, step by step

The form takes about a minute, and it works at any hour, which matters when a carrier wants the record before a start date.

1. Enter your full name exactly as printed on your licence.
2. Enter your 15-character licence number. Check it character by character against the card.
3. Give the email address the PDF should be delivered to.
4. Add your country, province or state, and postal or ZIP code.
5. Confirm your details and pay. Card details are handled by Stripe and never touch this site.
6. Watch your inbox. The completed five-year record arrives as a PDF you can save, print or forward to the carrier.

How long it takes

Most orders are delivered within minutes of payment. When a delay happens it is almost always because the name or licence number did not match the licence exactly, so a careful check before paying is the best way to avoid one.

What the record shows

A CVOR Driver Abstract summarizes safety information connected to a driver while operating heavy commercial vehicles in Ontario. Depending on your history it can include commercial vehicle inspections, safety-related convictions and collisions from the previous five years, together with your licence class, conditions, status, expiry date and medical due date.

Common problems and how to avoid them

• Name entered differently from the licence. Use exactly what is printed on the card, including a middle name.
• Mistyped licence number. It is long and easy to transpose. Read it back from the card before submitting.
• Ordering a carrier record by mistake. Level 1 and Level 2 abstracts are about a company, not a driver. Confirm the request names the driver abstract.
• An out-of-date copy. Many carriers want a record issued recently. Order it when it is actually needed rather than reusing an old one.
